The undersigned certifies that this application is for business purposes and not for personal, family or household purposes. The undersigned stands advised that any advance payment or security deposit is not refundable. To help the government fight the funding of terrorism and money laundering activities, Federal law requires all financial institutions to obtain, verify and record information that identifies each business customer opening an account. What this means for you: When you open a business account, we will ask for the name, address and other additional information that will allow us to identify the business. To verify this information, we may obtain reports from third parties, such as credit reporting agencies. We may also ask to see organization documents for your business. If your application for business credit is denied, you may be entitled to a written statement of the specific reasons for the denial. LEAF will send you a written statement of reasons for the denial within 30 days of receiving your request for the statement.
